Our staff would like to extend our warmest welcome to you. We are pleased you have chosen our office for your health care needs. Our desire is to present to you a comfortable and pleasant atmosphere for all your orthopaedic needs. Our mission is to provide quality orthopaedic care in a comfortable, caring atmosphere; serving each patient's individual needs, assisting the patient in accepting the responsibility for his/her orthopaedic and overall health, and maintaining a professional and efficient staff.
Appointments are made Monday through Friday. Please schedule your appointment on a day that will allow your stay in our office for up to 2 hours. We will strive to see you on time, however, emergencies may occur that may delay the schedule. If the physician is called out for an emergency, we will make every effort to notify and reschedule you.
We have a very important health history questionnaire to help your first visit run more smoothly . Please print and complete questionnaire (using a black pen) and bring it with you to your appointment. Your check-in time is 15-minutes prior to your appointment time. Patients arriving 15-minutes late may need to be rescheduled. Patients under 18-years old will need a parent or guardian to accompany them to all appointments.
It is important that you bring with you any x-rays or previous medical records you may have relating to the reason for your visit with us.
If you will be using insurance, please bring your current insurance card and a referral if your insurance requires one. Please be prepared to pay your co-payment at the time of your visit if your insurance requires one. A $15.00 billing fee may be added if co-payment is not made at the time of service. If you are a self-pay patient, we do ask that you be prepared to make a $100 initial deposit at your first visit. For your convenience, we accept cash, checks, Visa, MasterCard, and Discover credit and debit cards.
If your appointment is for a workman's compensation visit, prior authorization is required prior to your visit. Please have your employer's insurance information available for billing purposes.
